CT assessment of response to therapy: Tumor volume change measurement, truth data and error

This manuscript describes issues and methods that are specific to the measurement of change in tumor volume as measured from CT images and how these would relate to the establishment of CT tumor volumetrics as a biomarker of patient response to therapy. The primary focus is on the measurement of lung tumors, but the approach should be generalizable to other anatomic regions.

Eosinophil and T cell markers predict functional decline in COPD patients.

The major marker utilized to monitor COPD patients is forced expiratory volume in one second (FEV1). However, asingle measurement of FEV1 cannot reliably predict subsequent decline. Recent studies indicate that T lymphocytes and eosinophils are important determinants of disease stability in COPD. We therefore measured cytokine levels in the lung lavage fluid and plasma of COPD patients in order to determine if the levels of T cell or eosinophil related cytokines were predictive of the future course of the disease.

Dose to radiosensitive organs during routine chest CT: effects of tube current modulation.

The aims of this study were to estimate the dose to radiosensitive organs (glandular breast and lung) in patients of various sizes undergoing routine chest CT examinations with and without tube current modulation; to quantify the effect of tube current modulation on organ dose; and to investigate the relation between patient size and organ dose to breast and lung resulting from chest CT examinations.

Treatment of scleroderma-interstitial lung disease with cyclophosphamide is associated with less progressive fibrosis on serial thoracic high-resolution CT scan than placebo: findings from the scleroderma lung study.

The Scleroderma Lung Study (SLS) demonstrated significant treatment-associated improvements in pulmonary function and symptoms when patients with scleroderma-related interstitial lung disease (SSc-ILD) were treated with a 1-year course of cyclophosphamide (CYC) in a randomized, double-blinded, placebo-controlled clinical trial. This study examined thoracic high-resolution CT (HRCT) scans obtained during the SLS for treatment-associated changes over time.

Imaging the lungs in patients with pulmonary emphysema.

The emerging role of quantitative computed tomography (CT) in the evaluation of pulmonary emphysema is reviewed. CT is primarily used in the clinical setting to assess lung morphology visually. Increasingly treatments are being developed to treat patients with emphysema that require accurate quantitation of extent and distribution of the process. In addition, functional assessment can be made by inference of detailed anatomic correlates and by direct measurement of regional function using dynamic scan protocols. The aim of this review is to summarize the current role of imaging in the assessment of patients with emphysema. The review will focus on CT for the evaluation of morphologic abnormalities and on the potential role for the evaluation of functional abnormalities and treatment targeting.

Resection of Thyroid Cancer Metastases to the Sternum

The role of surgical resection in patients with metastatic thyroid cancer is not clearly defined. Reported is a case of concurrent thyroid metastases to the lungs and sternum treated with total sternectomy followed by radioiodine therapy. A comprehensive review of the literature was also performed to evaluate the characteristics of reported cases of sternal thyroid cancer metastases treated with surgical resection. Overall, we demonstrate that radical resection of sternal metastases can be performed safely even in patients with poor prognosis to achieve palliation and potentiation of radioiodine therapy for concurrent metastases.

Radiofrequency Ablation of Subpleural Lung Malignancy: Reduced Pain Using an Artificially Created Pneumothorax.

One of the main issues with radiofrequency (RF) ablation of the subpleural lung malignancy is pain management during and after RF ablation. In this article, we present a case that utilized a technique to decrease the pain associated with RF ablation of a malignancy located within the subpleural lung. Under CT guidance, we created an artificial pneumothorax prior to the RF ablation, which resulted in minimizing the pain usually experienced during and after the procedure. It also decreased the amount of pain medications usually used in patients undergoing RF ablation of a subpleural lung lesion.

Update on radiology of emphysema and therapeutic implications.

Emerging treatments require appropriate CT targeting of a selected lobe or lobes and target airways to obtain a successful response. CT scan is used in pretreatment planning to select patients and plan treatment strategy and posttreatment to confirm correct deployment of devices and assess treatment response. Increasingly treatments are being developed to treat patients who have emphysema who require accurate quantitation of extent and distribution of the process. Functional assessment can be made by inference of detailed anatomic correlates and by direct measurement of regional function using dynamic scan protocols. This article summarizes the current role of imaging in the assessment of patients who have emphysema.

Computer-aided detection of endobronchial valves using volumetric CT.

The ability to automatically detect and monitor implanted devices may serve an important role in patient care by aiding the evaluation of device and treatment efficacy. The purpose of this research was to develop a system for the automated detection of one-way endobronchial valves that were implanted for less invasive lung volume reduction.

Assessment of radiologist performance in the detection of lung nodules: dependence on the definition of "truth".

Studies that evaluate the lung nodule detection performance of radiologists or computerized methods depend on an initial inventory of the nodules within the thoracic images (the truth). The purpose of this study was to analyze (1) variability in the truth defined by different combinations of experienced thoracic radiologists and (2) variability in the performance of other experienced thoracic radiologists based on these definitions of truth in the context of lung nodule detection in computed tomographic (CT) scans.

Prevalence of tracheal collapse in an emphysema cohort as measured with end-expiration CT.

To retrospectively investigate the prevalence of tracheal collapse in an emphysema cohort. The occurrence of a large degree of tracheal collapse may have important implications for the clinical management of respiratory symptoms and air trapping in patients with emphysema.
